PATIENT SATISFACTION FORMS: Patients are invited to express their opinion on the quality of the service provided by the clinic by filling out a satisfaction form, which is available in the waiting room and at the reception. Patients can drop the forms in a dedicated box and can either leave the form anonymous or provide their names and telephone numbers. Forms are reviewed every month and submitted to the clinic chief medical officer.
THE USER HAS:
To collaborate with the clinic staff and follow the instructions to ensure that the procedure is correctly performed. It is advisable, though not mandatory, to submit the referral as well as any previous examination.
To inform the clinic about a possible pregnancy or any contraindications to the procedure also based on the content of the informed consent form.
To refrain from smoking and turn mobile phones off inside the clinic.
To arrive on time for the appointment.
To check at the time of payment if the amount charged and personal data on the invoice are correct.
To inform the clinic well in advance if the user cannot keep an appointment.
Complaints can be filed personally with the clinic chief medical officer after asking the reception staff for a meeting – the chief medical officer is always present during opening hours.
Complaints can be filed by mail too, sending a letter explaining the reason for the complaint and indicating an address for the reply.
They can also be filed by filling out a dedicated box in the patient satisfaction form.
